Ozarks Facility Request

Are you or your organization interested in using one of our campus facilities? If so, we have two convenient ways you can send your request to the Student Life office:

  1. Print the Facility Request Form* and return it to the Student Life Office.

OR

  1. Complete the online version of the Facility Request Form below. Note that items marked in bold are required.

Regardless of the method you use to submit your request, just remember that your request should be submitted at least 1 week prior to the date for which you are requesting a given facility. Facility requests are honored in the order in which they are received.
