Applying to JLC
To maximize your chances of being admitted to the JLC program, we encourage you to start the application process during the spring semester of your junior year or early in your senior year of high school.
To apply:
- Fill out the online application form
OR
print a copy of the application form*, complete it, and mail or FAX it to the JLC - Print out the following forms* and mail or FAX them to the JLC:
- Applicant's Statement
- Parent's Statement
- Physician's Report
- School Report
- Consent Form(s)
- Proof of Immunization
- Mail your $75 application fee to JLC.
